Theft Insurance Claim Process

Step 1: Call at 0172-5101366 for an immediate intimation of the theft (within 48 hrs of the incident).
Step 2: Get "Claim Intimation Number" (CIN) Registered.
Step 3: Intimate nearest Police Station on Theft and get Police Intimation letter "duly stamped & signed".
Step 4: Block SIM Card with your Telecom Operator and get SIM Blocking Letter "duly stamped & signed".
Step 5: Courier the documents to Gold key Insurance Brokers Pvt. Ltd., Chandigarh at the address mentioned on top left of this page.

Documents Required:

• Original Invoice copy


• Self-Attested Photo ID Proof of Claimant


• Police Intimation letter "duly stamped & signed" (DDR)


• SIM Blocking Letter from Service Provider "duly stamped & signed"


• If claimant is not a SIM owner, SIM owner photo ID proof to be submitted with rest of the documents.


• Insurance Claim Form (* marked are all mandatory information).


• Cancelled Cheque showing your account details (Including IFSC CodeandMICR Code).

CLAIM SETTLEMENT TAT:10 Working Days after submission of final documents.

Note:

• For timely processing and settlement of your claim you are advised to send "complete set of documents duly filled and signed" within 7 days of generating the claim intimation number.
